This form is used to collect proof of vaccination for Extended Studies students. The University will use this form to record vaccination status and/or determine who needs rapid testing.

All students are required to fill out this form in order to come to campus. A full course means you have had your final dose of either a two-dose or a one-dose vaccine series.

This form and the information it collects, or lack thereof, will be retained and may be referred to or relied upon in determining access to campus.

Protection of Privacy: Information provided on this form will only be used for the duration of the pandemic.
