This form is used to collect proof of vaccination for Extended Studies students. The University will use this form to record vaccination status and/or determine who needs rapid testing. All students are required to fill out this form in order to come to campus. Being fully vaccinated means it has been 14 days or more since you received a full course of a COVID-19 vaccination approved by either a Health Canada or a World Health Organization approved vaccine. A full course means you have had your final dose of either a two-dose or a one-dose vaccine series. This form and the information it collects, or lack thereof, will be retained and may be referred to or relied upon in determining access to campus. Protection of Privacy: Information provided on this form will only be used for the duration of the pandemic. While the information is retained it will be accessed only by those NSCAD University personnel who require access in order to determine whether the requirements to access campus have been met. The information will not be shared with any third parties.

If you received your vaccine in Nova Scotia you can retrieve this information here: For anyone who was vaccinated elsewhere in Canada: copies of your receipts are available on the relevant provincial website that you can find links to on the Government of Canada website. For anyone who was vaccinated outside of Canada: Proof can include vaccination receipts or letters from physicians, hospitals, or government agencies that include what specific vaccinations you have already received.
